As part of the Kidney Care Choices models launched by CMS, Fresenius Medical Care North America announced in a press release that it has partnered with more than 1,000 nephrology providers.

Through the partnerships, FMCNA intends to provide education and specialized care to slow the progression of kidney disease. Additionally, the program will encourage preemptive transplants and interventions to create a better plan for treatment of kidney disease.

"We appreciate the more than 1,000 nephrology providers who have chosen to partner with us in this important government value-based program to improve care for people living with late-stage kidney disease and kidney failure," David Pollack, president of FMCNA's Integrated Care Group, said in a press release.

Most of the physicians FMCNA partnered with are members of InterWell Health. The partnerships are also designed to reduce hospital admissions.

"We are committed to delivering better outcomes while reducing total costs to our health care system,” Pollack said. “Our innovative solutions have shown we can truly make a difference as we work to intervene earlier to treat kidney disease under new value-based models of care."
