DaVita works with kidney doctors, transplant providers to launch 11 programs across the US

DaVita Integrated Kidney Care has joined forces with more than 1,000 kidney doctors, transplant providers, hospice providers and advanced care practitioners to launch 11 value-based care programs across the U.S.
According to a company press release, the 11 programs are part of the federal value-based care demonstration, Kidney Care Choices (KCC) model. The demonstration is planned to last for 5 performance years. The KCC includes a Comprehensive Kidney Care Contracting (CKCC) option, in which DaVita and its partners are participating.
"We're honored to be part of this program and be the partner of choice for so many purpose-driven providers who, like us, are committed to setting a new standard for kidney care," David Roer, MD, chief medical officer for DaVita Integrated Kidney Care, said in the release. "We've honed our approach to integrated kidney care for over a decade and, together, we believe we can improve the lives of our shared patients."
The CKCC is designed to allow dialysis centers and nephrologists to create “kidney-focused [ACOs] to manage care for Medicare patients.” Additionally, the plans include incentives to manage care for Medicare patients with CKD stages 4 and 5.

"We can help patients better plan for their future kidney care needs –whether that's a preemptive transplant or an optimal start on dialysis at home – and improve their overall health while helping reduce the cost to our health care system."
