Vantage, FMCNA seek to improve outcomes for patients with kidney disease

Vantage Health announced its partnership with Fresenius Medical North America and a new care agreement aimed at improving outcomes for patients with late-stage chronic kidney disease, according to a company press release.
The companies are focused on value-based care, seeking to reduce unnecessary hospitalizations, improve health outcomes and lower the cost of care for patients with kidney failure or end-stage renal disease.
“The collaboration between Vantage and Fresenius Medical Care North America was developed to improve the quality of life for Vantage members living with renal disease,” Gary Jones, MD, CEO of Vantage Health Plan, said in the release. “By working together, we can better coordinate care and expand access to treatment for our members.”
This agreement will align health care reimbursement with quality performance to deliver greater value to Vantage Health Plan members, as well as give members access to more than 2,600 FMCNA dialysis centers and home dialysis options to help them lead fuller, more active lives, according to the release.
“This partnership will help us deliver more coordinated care and personalized treatments for people living with late-stage kidney disease,” David Pollack, president of FMCNA’s Integrated Care Group, said in the release.
