Blue Cross and Blue Shield of Minnesota announced it will partner with DaVita Inc. to launch its integrated kidney care VillageHealth program in the Blue Cross Minnesota network.

According to a press release, VillageHealth will allow Blue Cross members with chronic kidney disease or end-stage kidney disease to seek earlier and better care at a reduced cost. The new program promises improved kidney disease screening, high-risk identification and patient education.

“We have a significant opportunity to help delay the progression of kidney disease and better meet the complex health needs of those with kidney failure,” Mark Steffen, MD, chief medical officer for Blue Cross, said in the release.

DaVita’s program will also help patients make more active choices in their care options, such preemptive kidney transplants and in-home or in-center dialysis.

“The earlier we identify patients and coordinate their care, the more impact we can have on important outcomes, such as slowing progression, transplantation or choosing home dialysis,” Jeff Giullian, MD, chief medical officer for DaVita, said in the release.
