Monogram Health expands home kidney program for Cigna Medicare Advantage members

Monogram Health, a company focused on providing management services for patients with kidney disease, has expanded access to its in-home kidney care management program, allowing for more Cigna Medicare Advantage members to join it.

According to a press release, the program, previously available in a handful of states, is now available in several more regions across the United States. These include Texas, Pennsylvania, Illinois, North Carolina, South Carolina, New Jersey, Delaware, Maryland, Missouri, Kansas, Colorado and Washington D.C.

"Monogram's care managers provide qualifying Cigna customers across the country with in-home, personalized services that help them access the health care they need and deserve," Bill Frist, MD, former U.S. Senate Majority Leader and Monogram Health's board chair, said in the release. "Our organizations share a commitment to evidence-based renal care and improved health outcomes for those people with chronic kidney disease and end-stage renal disease."

The release noted that under the program, nurse case managers and social workers conduct in-home visits, while physicians develop personalized plans to reduce hospitalizations. For further personalization of care, the release stated, plans address social determinants of health.

"Working with Monogram Health, Cigna is taking a personal, individualized approach to raising the quality of care for our customers with CKD and ESRD," Gina Conflitti, MD, chief medical officer for Cigna Medicare Advantage, said. "Cigna wants our customers with kidney disease to know all of their care options. We will work to help them receive evidence-based treatment that maximizes their health and quality of life."
