Fresenius launches care coordination program for patients with kidney disease

At the start of National Kidney Month, Fresenius Medical Care has launched a program focused on improving care coordination for patients with chronic kidney disease.
More specifically, the program is designed to help slow disease progression through earlier interventions, as well as patient education on disease management and treatment options for kidney failure, according to a press release.
These goals, according to the release, will be achieved though “close partnerships” with physicians and physician networks.
“Our KidneyCare:365 program is a critical part of our value-based care services, created in close collaboration with our physician partners at InterWell Health,” David Pollack, president of Fresenius Medical Care North America’s Integrated Care Group, said. “This program will help us better coordinate care for thousands of people with kidney disease in partnership with health plans and the federal government, as we work to improve outcomes and lower the total cost of care.”
The release noted the program has already been implemented by a variety of health insurance providers across the country.
“Too many patients with chronic kidney disease do not receive a timely referral to a nephrologist,” Terry Ketchersid, MD, MBA, chief medical officer for FMCNA’s Integrated Care Group and co-chief medical officer for InterWell Health, said. “By working closely with our nephrologist partners and health plans, we are confident we have the ability to improve lives and outcomes for this vulnerable population.”
