Blue Shield of California has partnered with Cricket Health to develop a “personalized and comprehensive care coordination program” for its members with late-stage chronic kidney disease or end-stage kidney disease.
According to a press release, Cricket Health will provide Blue Shield members with a multidisciplinary care team consisting of nurses, pharmacists, social workers, dieticians and patient mentors. The team, available online and by phone, will work with the patient’s primary care physicians, nephrologists, and other specialties to develop a personalized care plan. The care team will then inform patients of their treatment options and assist in deciding what best fits their lifestyle and goals.
“This collaboration will fundamentally change what a kidney disease diagnosis means for patients, putting them in control of their kidney-care journey,” Arvind Rajan, CEO of Cricket Health, said in the release. “Our comprehensive approach to chronic kidney disease and end-stage renal disease care focuses on preventive care, slowing progression of the disease, and when needed, getting the treatment that’s right for them. Together with Blue Shield, we will support their members with a multidisciplinary care team that reaches patients in the safety and comfort of their homes.”
Seth Glickman, MD, chief health officer for Blue Shield of California, also weighed in on the collaboration.
“We recognize that for too long, the health care system in the U.S. has fallen short of providing patients who suffer from kidney disease – an estimated 37 million Americans – with the highest quality health care at an affordable cost,” he said. “By working with Cricket Health, we can reimagine kidney care and expand health care options for members with late-stage chronic kidney disease and end-stage renal disease so they can live the healthiest lives possible.”
The release added that the program is one component of Blue Shield’s Health Reimagined initiative, which seeks to better the health care system, in part, by addressing health inequities and expanding access to quality kidney care.
The program is available to all members enrolled in Blue Shield’s fully insured preferred provider organization benefit plans.
