DaVita announced this week that the company’s Health Solutions subsidiary would be renamed Vively Health, continuing with its mission to provide in-home primary care for chronically ill patients — a group it refers to as their most vulnerable patients or MVPs.

“Vively means 'full of life' or 'lively,' so it evokes the way we want to help our patients feel,” Hank Schlissberg, Vively president, said in a company press release. “While we now operate as a standalone subsidiary of DaVita, Vively will continue to build on the strong roots established by DaVita Health Solutions as we transform care for MVPs.”

Under full risk arrangements, DaVita Health Solutions' community-based, physician-led care teams deliver medical, behavioral, social and palliative care to chronically ill patients within their home. The company's house calls programs have enhanced the lives of thousands of patients with a 91% satisfaction rating; 35% to 40% reduction in hospitalizations; a 10% to 15% reduction in emergency room visits; and a 15% to 20% reduction in cost of care, according to the release.

Vively Health announced on Oct. 29 that it signed a 5-year contract with Cerner Corporation, a health platform and innovation company, to integrate Cerner's technology across its home-based offerings, including its electronic health record, population health management platform and consumer framework, which is designed to help lower the cost of care by supporting care teams to better address the needs of MVPs.
