High-dose influenza vaccine may lower hospitalization rates in patients on dialysis
Patients on dialysis who received a high-dose influenza vaccine were associated with lower rates of hospitalization compared with patients who had a standard-dose influenza vaccine, according to recently published results.
Dana C. Miskulin, MD, and colleagues compared hospitalizations and deaths of patients who received maintenance in-center hemodialysis or PD who received an influenza vaccine at their dialysis facility during the 2015 to 2016 and 2016 to 2017 influenza seasons. Primary outcomes included all-cause hospitalization and all-cause death that occurred at least 14 days after vaccination. Researchers collected the type of inactivated influenza vaccine administered, including standard dose trivalent, high dose trivalent and standard dose quadrivalent.
Results showed 31%, 61% and 8% of patients received standard-dose trivalent, standard-dose quadrivalent and high-dose trivalent influenza vaccines, respectively, between Sept. 1, 2015 and Dec. 31, 2015. Researchers found the standard-dose trivalent vaccine had an adjusted rate of first hospitalization of 8.43 per 100 patient months compared with the standard-dose quadrivalent vaccine at 7.88 per 100 patient months and the high-dose trivalent vaccine at 7.99 per 100 patient months. Researchers also noted adjusted rates for death of 1, 0.97 and 1.04 per 100 patient months for standard-dose trivalent, standard-dose quadrivalent and high-dose trivalent vaccines, respectively. However, the differences were not significant, according to results.
The 39% of patients who received the quadrivalent vaccine in 2016 had adjusted rates of first hospitalization of 8.71 per 100 patient months and of 0.98 per 100 patient months for death, results showed. Researchers found adjusted rates of first hospitalization of 8.04 per 100 patient months and of 1.02 per 100 patient months for death among 61% of patients who received the high-dose trivalent vaccine. Patients who received a high-dose influenza vaccine had a significant reduction in hospitalization, according to results, but no significant association with death.
