Fresenius Medical Care, Aetna provide coordinated care management for ESRD patients

Fresenius Medical Care and Aetna have launched a collaborative care model designed to improve the quality of care, and lower costs, for Aetna’s Medicare Advantage members who have end-stage renal disease.

Fresenius Medical Care North America’s nationally-recognized care management program is accredited by the National Committee for Quality Assurance, and creates a new approach for the overall delivery of care focused on the unique needs of dialysis patients. Through this collaboration, Aetna nurse case managers will work closely with FMCNA staff on quality and care management for Aetna Medicare Advantage members who are receiving care at an FMCNA clinic. Aetna and FMCNA will work together to improve the health outcomes of these patients by boosting adherence to best practices and treatment plans.

The development of the collaborative ESRD program with Aetna is an extension of the CKD program launched in 2012, Fresenius Medical Care spokesperson Jon Stone told NN&I.

"The CKD program provides education, training and care management for Aetna's members with chronic kidney disease," said Stone. "The collaborative ESRD model of care which we just announced leverages the entire suite of services that FMCNA offers, within a coordinated system, to serve Aetna's Medicare Advantage members who receive dialysis care in our clinics. It also engages patients in a way that is intended to improve their health outcomes while helping to reduce the total cost of care. FMCNA Nurse Care Managers assigned to each Aetna Medicare Advantage member on dialysis will work closely with the member and his or her physician and hospital providers to help break down communication barriers and create a more seamless and efficient health delivery experience."
