Dialysis Clinic Inc. to use new platform for patient engagement, care management

Dialysis Clinic Inc. has signed an agreement with cloud-based care management technology company RoundingWell to use their patient engagement and care management platform, designed to increase effective communication between patients, dialysis providers, andthe interdisciplinary team.

Conversations between the two Nashville based organizations began over a year ago when DCI started working on the ESRD Seamless Care Organization (ESCO) application, DCI said in a statement.

“We were evaluating tools that would aid the communication process from patient to provider and among providers," said Doug Johnson, MD, DCI Vice chairman of the board. "Essentially the tool needed to achieve two goals. It needed to offer an engaging patient-communication platform that allowed us to share educational information and assess a patient’s current needs. At the same time, we required the solution to provide essential feedback to the care coordinator that could efficiently be shared with the interdisciplinary team,” 

DCI chose RoundingWell among four candidates vying for the contract.

The customized patient engagement and care management platform provides:

  • Proactive online health check-ins to engage patients
  • Patient risk identification and stratification
  • Automated care-coordination pathways, tailored to reflect the needs of patients within a specific market
  • Consistent internal and external communication among caregivers

DCI said the system is expected to go live with a first group of patients in the Nashville market in October.
