DaVita HealthCare Partners, SCAN Health Plan expand VillageHealth C-SNP into Los Angeles, Orange Counties

DaVita HealthCare Partners Inc. and SCAN Health Plan have expanded their chronic condition special needs plan (C-SNP) for individuals with end-stage renal disease into Los Angeles and Orange Counties, beginning Jan. 1, 2014. The HMO plan, called VillageHealth, has begun enrolling members, DaVita HealthCare Partners said in a news release.

“We are excited to expand this plan that combines DaVita’s kidney care expertise and HealthCare Partners extraordinary care management infrastructure to create a comprehensive product for patients with kidney failure,” said Todd Ezrine, DaVita VillageHealth general manager. “This plan has proven to be very appealing to ESRD patients.”

VillageHealth is designed specifically to address the unique needs of kidney care patients, according to DaVita HealthCare Partners. This plan is available to ESRD, kidney transplant and post-transplant patients who are Medicare eligible. SCAN and DaVita have been offering the VillageHealth C- SNP as a Point-of-Service plan in Riverside and San Bernardino counties since 2006.

“Chronically ill patients have special needs, and dialysis in particular can be a difficult process for both patients and their families,” said Romilla Batra, MD, SCAN corporate medical director. “Coordinated care plans like VillageHealth can lead to better patient outcomes and overall improved quality of life.”

VillageHealth members receive all care management and dialysis services from DaVita HealthCare Partners’ team, which includes nephrologists, nurse practitioners, care managers and pharmacists.

“Our patient members choosing this plan receive a strong wellness program, complex care coordination as well as new benefits including services like comprehensive dental coverage, vision coverage and unlimited support with regard to their nonemergency transportation needs,” said Jeff Flick, vice president of national government programs for DaVita HealthCare Partners.

The interdisciplinary care team will coordinate the patient’s total care and help the patient navigate the various care settings such as dialysis centers, hospitals and specialist offices, according to the news release. Plan members receive their entire Medicare professional and hospital services as well as their prescription drug benefit through VillageHealth.
