The American Association of Kidney Patients has added a weekly food and nutrition log, to its online health record tool. AAKP  myHealth is a free online tool that allows patients to create a secure online profile to monitor their health care, including lab values, medications, doctor appointments, immunizations, contact information/emergency contact information. The nutrition tracker is designed to help patients achieve their diet and nutrition goals. Patients can track their nutrient intake including sodium, potassium, phosphorus, protein, fat, saturated fat, carbohydrates and calories.
