DENVER — In a Healio video exclusive, Stefan D. Holubar, MD, MS, discusses the decades-long controversy surrounding preoperative use of biologics and risk for postoperative complications in patients with inflammatory bowel disease.

According to Holubar, IBD surgery section chief and the director of research for the department of colorectal surgery at Cleveland Clinic, analysis of data from the PUCCINI trial and National Surgical Quality Improvement Program (NSQIP)-IBD Collaborative showed no association between preoperative biologic exposure and postoperative complications, infectious complications or anastomotic leaks.

“Most of us who know this literature well base our recommendations on these two very large, well thought-out studies,” Holubar said. “The best available evidence that we have to date is that biologics do not appear to be associated with postoperative complications, much less infectious complications. It’s safe to continue them throughout the perioperative period.”
