Kurlander and colleagues conducted a study, published in Annals of Family Medicine, in which they assessed prescribing practices and barriers to the use of PPI gastroprotection within and across specialties, based on interviews with physicians in four specialties at a single institution. They then conducted a thematic assessment of the barriers, organized around a theoretical domain framework.

The researchers found that across the specialties none consistently prescribed PPI gastroprotection, and the main barriers included knowledge, decision processes and professional role.

“Based on the multitude of barriers that exist to prescribe PPI gastroprotection, any intervention in the future is going to have to address these barriers,” Kurlander said. “These will have to be multi-component interventions. We know that to change clinician behavior you have to effect their capability, opportunity and motivation to practice evidence-based care.”
