VIDEO: The right transition to adult care empowers patients
CHICAGO — In this exclusive video from the Interdisciplinary Autoimmune Summit, Anita Afzali, MD, medical director of the Inflammatory Bowel Disease Center at The Ohio State University Wexner Medical Center, discusses how physicians can help adolescent patients transition to adult care.
“This is relevant for all chronic medical conditions or immune mediated diseases,” Afzali told Healio Gastroenterology and Liver Disease.
Important factors to consider when preparing a patient to move into adult care include introducing the concept of transition early on, evaluating transition readiness and sharing medical summaries among all the relevant parties.
Afzali said it is also important for the adult care team to communicate with the patient’s previous, pediatric care team to discuss what steps will be taken moving forward.
“It’s very important to empower our young adults with their chronic medical diseases,” Afzali said.
