Pregnancy loss, preterm delivery reduced with levothyroxine therapy

DENVER — Levothyroxine therapy reduced pregnancy loss, preterm delivery and offspring with low birth weight or low Apgar score for women with subclinical hypothyroidism negative for thyroid peroxidase antibodies, researchers reported at the Annual Meeting of the American Thyroid Association.

However, they found a higher incidence of gestational diabetes and premature rupture in membranes in women prescribed levothyroxine. Read more.

Patients with Hashimoto's thyroiditis require similar T 4 levels regardless of body weight

DENVER — Thyroxine requirements in Hashimoto’s thyroiditis are similar for patients with normal weight or overweight, study data show.

“The analysis of thyroxine requirement in a population of adult hypothyroid patients with Hashimoto’s thyroiditis revealed that the dose needed by overweight patients is similar to that in normal-weight patients,” Camilla Virili, MD, of the department of medicosurgical sciences and biotechnologies at Sapienza, University of Rome in Italy, told Endocrine Today. “On the contrary, obese hypothyroid patients require progressively lower thyroxine dose as BMI increases.” Read more.

Thyroid cancer overdiagnosis raises questions about overtreatment

DENVER — The “epidemic” in thyroid cancer recently reported by a number clinical journals has raised questions about overdiagnosis of thyroid cancer and overtreatment of papillary microcarcinoma, according to a presenter at the 86th Annual Meeting of the American Thyroid Association’s opening session.

Ian D. Hay, MD, PhD, FACE, FACP, FRCP, professor of medicine and endocrine research at the Mayo Clinic, reviewed data from a number of previously published studies evaluating the incidence of thyroid cancer, particularly papillary thyroid cancer (PTC) and papillary thyroid microcarcinoma (PTM). Recommendations needed for TPOAb cut-off levels during pregnancy

DENVER — The presence of thyroid peroxidase antibodies during pregnancy may affect thyroid function, but no recommended cut-off ranges to identify thyroid peroxidase antibody–positive women are currently available.

However, the draft 2016 American Thyroid Association guidelines on thyroid and pregnancy states that treatment can be considered for lower thyroid-stimulating hormone concentrations in thyroid peroxidase antibody (TPOAb)–positive women as compared with TPOAb–negative women. Read more.

Hypocalcemia management strategies differ among physicians after thyroidectomy

DENVER — Surgeons differ in strategies to prevent and manage post-thyroidectomy hypocalcemia, according to findings presented at the Annual Meeting of the American Thyroid Association.

J. Pieter Noordzij , MD, professor in the department of otolaryngology–head and neck surgery at Boston Medical Center, Boston University, and colleagues evaluated survey responses from 203 members of the ATA and 129 members of the International Association of Endocrine Surgeons to determine their practices for prevention and management of hypocalcemia after thyroidectomy.
