The Joslin Diabetes Center was awarded a $5 million, 3-year grant from the CMS Innovation Center to augment its community-based diabetes outreach program.

The Boston-based center is the world’s largest diabetes research and care organization, conducting field testing and providing risk assessments to low-income or uninsured populations.

The grant will allow the program to partner with Penn State University and New Mexico State University Extension educators, as well as Providence Health Services Inc. in the Washington, D.C., area.

“Joslin has worked for over a decade with our community-based colleagues to develop and refine this program, and we are excited and honored about the opportunity to partner with CMS in expanding its reach and impact,” Richard Jackson, MD, project director for “On the Road,” and director of medical affairs of health care services at Joslin, said in a press release.

By working with additional partners, Joslin aims to target at-risk populations and better patients’ diabetes management.
