‘Inside-out’ technique for ventral hernia repair is easy, reliable

Researchers found no hernia recurrence in patients who underwent repair for a primary ventral hernia with an underlay “inside-out” technique.

The “inside-out” technique was performed on 23 patients at a single center and follow-up lasted a median of 24.5 months to evaluate postoperative complications and hernia recurrence.

The surgeons used a Carter-Thomason (Cooper Surgical) suture passer which allows for safe passage of preplaced sutures on the mesh from inside the abdominal cavity through the anterior rectus sheath, according to researchers.

The underlay of the mesh is fast with this technique and tension distribution around the repair is improved, which can reduce the risk of hernia recurrence.

The researchers found a hernia recurrence rate of two out of 23 or 8.7%, which was deemed acceptable.

Of the recurrences, one patient underwent repair of a recurrent ventral hernia and the other had significant loss of domain and required an inlay mesh.
