The Secondary Manifestations of Arterial Disease trial evaluated the relationship between epicardial adipose tissue and presence, distribution and density of coronary artery calcium in high-risk patients.

Results: A 1 standard deviation lower attenuation (5 Hounsfield Units) was associated with greater odds for men of being in a higher CAC class (OR = 1.9; 95% CI, 1.14-3.19) independent of age, prior CABG, BMI and epicardial adipose tissue volume, but the relationship was not significant for women (OR = 1.07; 95% CI, 0.41-2.75). A 1 standard deviation lower attenuation was not related to distribution of CAC, but elevated odds of being in a higher tertile of CAC density per plaque (OR = 1.77; 95% CI, 1.18-2.66).
