DIGAMI Trial

Trial Details:

Acronym: Diabetes and Insulin-Glucose infusion in Acute Myocardial Infarction

Purpose: To determine if strict control of glocuse levels in diabetic patients during acute coronary syndromes improves outcomes.

Format: Double-blind, multicenter, randomized control trial

Treatment Group: Insulin IV infusion (insulin drip) followed by subcutanous insulin

Control Group: Routine diabetes therapy

Number of Patients: 620

Inclusion Criteria:

  • Suspected acute coronary syndrome within 24 hours
  • Diabetes mellitus
  • Blood glucose > 11 mm/L (even if formal diabetes diagnosis not present)

Exclusion Criteria: 

  • Serious illness
  • Refusal to participate

Follow-up: Average of 3.4 years

Primary Endpoint: All-cause mortality

Secondary Endpoint: None

Summary:

Evaluated intensive glucose control in diabetic patients with acute coronary syndromes. There was a reduction in long-term mortality in the intensive treatment group. This was not reproduced in the DIGAMI 2 trial.
