Acute mitral valve regurgitation frequently does not cause a murmur. Since the left atrium has not had sufficient time to structurally adapt to the sudden increased pressures, the left atrial and left ventricular pressures equalize quickly in early systole resulting in only a short, early systolic murmur or no audible murmur at all.
