Patient referred for visual loss after cataract surgery

An 85-year-old woman was referred urgently for poor vision on the first postoperative day after cataract extraction of the right eye. She reported moderate pain the night of surgery, which resolved by the following day. Her preoperative vision was 20/70, and on postoperative day 1, her vision was limited to counting fingers. The referring ophthalmologist reported an uncomplicated cataract operation.

Patient has visual loss 1 day after cataract surgery

Jeffrey Chang Vivek Chaturvedi A 69-year-old woman was referred to the New England Eye Center for decreased vision on postoperative day 1 after uncomplicated cataract extraction in the right eye. The patient’s ocular history was unremarkable other than uncomplicated cataract surgery in the left eye 2 months earlier. Medical history was significant for hyperlipidemia, gastroesophageal reflux disease, osteoporosis, and breast cancer status post lumpectomy and radiation therapy. Medications included atorvastatin, omeprazole, risedronate and exemestane. Allergies included penicillin, sulfas, iodine and intravenous dye. Family history, social history and review of systems were unremarkable.

Woman referred for recurrent bilateral ptosis

A 49-year-old woman was referred by her primary care provider for further management of long-standing ptosis, with new onset of decreased vision, eye pain and an inability to close her right eye. She had a history of multiple eyelid surgeries, including bilateral levator advancement 25 years ago, lower eyelid external tarsoaponeurectomies 20 years ago and multiple revisions for each of these surgeries. Six months prior, she underwent bilateral frontalis suspensions. Over the past several months, she developed increasing eye pain despite aggressive lubrication, along with decreased vision of the right eye.

HIV patient presents with painful unilateral visual loss

A 47-year-old Hispanic man presented with a 4-day history of painful unilateral visual loss. He reported “everything seems to appear darker” and suffered from a “boring pain” in the left eye. Ocular history was significant for an episode of central serous chorioretinopathy 9 years earlier in the left eye that resolved with observation.

Woman referred for progressing unilateral glaucoma

A 42-year-old woman was referred by her primary ophthalmologist for management of progressing glaucoma. She had glaucoma in the right eye for 8 years. Until recently, IOP in her right eye had been adequately controlled by topical aqueous suppressants.
